Booking form to send via fax - to be filled by hand.
The vacationers are admitted upon paid sanatorium package. on arrival the vacationer should possess the following documents :

sanatorium package ;
- personal ID (Birth Cert. for children);
- vaccination card ;
- medical statement ;
- power of attorney for receiving sanatorium pacakage (or transferrable sanatorium package of your agency/firm).
